So yes, transforming would be clear due to it not being game-breaking! The vehicle form would just have to be something setting-appropriate and size-appropriate. My thought is that First Aid's size change could be accounted for by something that isn't necessary smaller, perhaps just less mass.

After some poking around, any of these carriages or wagons would be fine! Seeing a carriage fly through town without a horse might startle the locals, but they will probably ultimately chalk it up to "magic" and move on with their day after a brief spook. The carriage can also retain some of First Aid's design elements so that it's clear he is a medical vehicle!
