So, considering that we generally do not allow characters that are minors in Pumpkin Hollow, it would probably be best if he appeared at his real age of 18. That way Shiro could be compliant with the rules, complete with a convenient lore excuse!
As for his sickness, we'll leave that part up to you! You could say the illness isn't able to follow him through death and have him be well, have a milder version of the illness, or have him fully retain it and potentially die from it repeatedly--- whatever sounds the most fun to play. There is medicine, but because it's a semi-steampunk setting, there'd be roughly Victorian-era medical technology + a little magic, so our advice would be to keep that in mind when deciding.
